Join South East Wales Independent Practitioners Group

Membership of the group is open to all practitioners in the area, from GDP's to community staff and those in hospital posts. If you would like to join then we will require you to complete an application form, which can be downloaded on this page.

Membership of the group costs £10 per month. The membership fees are used to subsidise the meetings and pay for the running costs of the group, e.g. this website.

Please download the form in the format that suits you. Fill out the form in black ink, sign it (signature must be provided) and then send it to the following address along with your completed Direct Debit Mandate form.
